How to Complete Your Patient Forms
To make check-in as quick as possible, we will send your New Patient Registration packet by email or text before your first visit. You can complete it at home and bring the finished paperwork with you. If you prefer, arrive about 15 minutes early and our team will provide the forms at the front desk.
What to Bring to Your First Appointment
- Completed patient forms (or arrive 15 minutes early to fill them out in the office).
- A valid photo ID and your dental insurance card, if applicable.
- A list of current medications, dosages, and known allergies.
- Names and contact information for any previous dental providers you would like us to coordinate with.
